Description:

Under the California Advancing and Innovating Medi-Cal Initiative (CalAIM), DHCS transitioned Cal MediConnect plans to Medi-Medi Plans in seven counties, managed by the same health plans that offered Cal MediConnect.

The Medi-Medi plans meet integrated care coordination requirements, have integrated member materials, and have membership limited to individuals dually eligible for Medicare and Medi-Cal. Dually eligible members have a higher prevalence of chronic conditions than many other Medi-Cal populations, and must navigate across several delivery systems. Medi-Medi Plans help coordinate across those delivery systems.

All Cal MediConnect members were automatically transitioned to corresponding Medi-Medi Plans under the same parent organization on January 1, 2023, along with some dual eligible members in "D-SNP Look-Alike" plans. Those transition populations, as well as additional enrollment selections by members, combine to make the total Medi-Medi Plan enrollment in 2023.
